Overview

Tom is a Credentialed Diabetes Educator who has also worked as a clinical pharmacist for over 25 years. He holds a Master of Clinical Pharmacy from the University of Queensland and a postgraduate Certificate of Diabetes Education from Deakin University.

With experience working across a broad range of backgrounds, he has practised across most patient settings, including community, hospital, and nursing homes. He is dedicated to supporting individuals living with diabetes in diverse and complex community environments.

Tom speaks English, Russian, and Arabic.

Tom accepts patients with and without referrals. Medicare care plans from GPs are accepted and bulk-billed. Patients without a care plan will incur a fee of $75 per session.

He can provide education and support for you to:

  • Understand diabetes.
  • Understand the role of diabetes medications.
  • Set and achieve your health goals.
  • Feel confident and supported in managing diabetes.
  • Monitor blood glucose levels and screen for risk factors.
  • Understand the use of Continuous Glucose Monitoring (CGM).
  • Learning to give yourself diabetes injections.
  • Start and titrate insulin.
  • Have a Sick Day Management plan – Know what to do if you are unwell.
  • NDSS (National Diabetes Services Scheme) registration.
